Every player must complete the required 7v7 LeMoyne Medical Waiver Form and give it to your coach/manager to be presented at registration the day prior to the event.

Note: 7v7 LeMoyne Medical Waiver Form is required and not player will be permitted to participate without this form on file with tournament headquarters. Each team will submit the Medical Waivers at the time of walk up registration.
